Damaged or dirty
I played U4 for days, i haven’t played it a month or something. When i put the cd in, it says that de disc is dirty or damged but its clean and there are no scratches. I tried to update my ps4 in safe mode, and i dont want to reset my playstation because then my other Uncharted games are gone ;-; please help

I'm having the exact same issue...very frustrating. Sometimes I can trick the game into continuing by putting ps4 into rest mode immediately when I receive the error, and then booting it back up. This only works a fraction of the time, however, and the game will often crash again.

Same problem for me, updated to latest version of U4 to Ver 1.32, disc is clean no scratches, rebuild database, unstalled U4 and reinstalled from disc and still getting the error.
