First, make sure Nadine is close enough to Chloe when clicking the magnifying glass prompt.
Anyway, my best guess is that your save file(s) is glitched.
Try making a new manual save and then load it.....or restart the chapter and make a new save file.
What relevance does the PS3 have to Uncharted 4?
The game isn't available for that system.
Why post the same message four times?
Is your ps4 system and the game updated to the latest patch?
Disc (used or new?) or digital download?
Time since install?
System free space?
How/When does the game crash, i.e. randomly or always at the same point/area? Are you getting any error codes?
What you're describing sounds like corrupted game data. On a digital download this is usually caused by a failed download (i.e. a poor internet connection) and/or a failing hdd/sdd.
Do other games work like they should on your ps4?
After initializing your ps4, did you let the game install properly/fully "over night", so to speak, or did you start playing as soon as the game/ps4 let you?
The only thing I can think of is that your profile file could be corrupted/glitched. Make a backup of it and then delete it and try again.
Alt. check if you possibly have more than one U4 profile file.....
Part of being a gamer is trying/testing things (like all the different buttons, etc.) even if the game hasn't outright told the player about them yet.
Game devs like Naughty Dog generally don't want to be "too much on the nose" with the tutorials. Hence the game only tells you about the basics. The rest the player is expected to figure out for themselves by trying and testing things, which is more rewarding.
Well, you could just test if Hometown works in a private match set up, even-though you can't get the trophies.....
The only solution I know of is to re-install that particular map pack again, just to be sure.
I doubt you'll get a response from Naughty Dog here (I haven't seen one in over a year), plus this support/feedback page is not for the ps3 either.

You can't start a private match on the Hometown map, or it doesn't come up in the normal rotation, "find match (parties allowed)", even if you select "find match (dlc)"?
Make sure all 8 party members has the map pack installed correctly; i.e. locate the map pack in the xmb download list (alt. view downloads in the ps3 ps store), then download and install it again.

If your ps3's storage is full you shouldn't use restore in safe mode. Deleting game data to free up storage space is a much easier and less headache inducing solution.
Anyway, enter your voucher code for the Uncharted 3 network/online pass or check your download list in the xmb for it, and then download/install it again.

That's very doubtful.
Most games' online/mp servers get shut down after 5 to 10 years. So, anyone joining (too) late can't get the platinum.
The best bet after September 3rd will be a fan-server project, or similar, like Demon's Souls.
OK then, maybe you have a corrupted/damaged save file.
Backup and then delete all your save files. Then make a new fresh save file before attempting the powerpyx method.
...and/or your profile file is glitched (or you have more than one......). Backup your old one(s) and then delete it/all. Reboot the game and the game should make a new one.

The UK/European one doesn't work together with the US version of the game.
Yes, if you can't find an update for the game you should have v1.11 installed (triangle -> information, should also tell you the version you've got installed, or check the tlou-items in the game data utility folder).
To make sure what version/region your game is for you need to check the product code stated either on the disc, spine of the cover, or similar (e.g. BCES-01585, BCUS-98174, etc.).
Clicking the link to the European web store will only inform the ps store where you're located in the world. Like I said, if you want to try to purchase and download the European free bundle you need to open the link in an incognito window (or similar), or use a vpn service.
To check your psn account country/region you either need to log in on the web store and check your personal details or on the ps3's account settings/details.
Also, after doing some more research on the map packs, I found out that the two maps in the Treacherous Territories map pack (Beach and Financial Plaza) were first included in the v1.09 patch for the ps3 back in late August 2014: https://www.naughtydog.com/blog/the_last_of_us_patch_103_and_109_free_maps
So, if you're having problems using Factions after getting a network/online pass to work, I suggest either delete the latest patch (v1.11) and then re-apply it, or take a look in your download list (or view downloads) and look for the map pack there and re-download/install it.

OK so, you're in the US (I'm in Europe, btw), is the game for the US and your psn account is set up for the US?
Afaics, there is no possibility to buy the network/online access pass for the ps3 version in the European ps store any longer (which makes sense since it's included in the free bundle).
It is still available for purchase in the us ps store though.....so, it's hard to tell if this is deliberate or a mistake on the US or European ps store's part.
Maybe contact Sony and ask them why the regional differences....
When it comes to the map packs; I have three of them in my download list; the two you mentioned plus the treacherous territories map pack (which is about 142 MB in size)
Maybe try downloading the free Double Map Bundle on the US store, or check if you can access anything through the game's menus.
Also, the European free ps3 add-on bundle is 692 MB: https://store.playstation.com/en-gb/product/EP9000-BCES01585_00-TLOUADDONSBUNDLE . The US equivalent is only about 550 MB (which makes for a difference of 142 MB, which is the size of the third map pack I mentioned)!
Try to purchase the European bundle and see what that adds to your download list(s). You'll probably need to open it and then log-in on an incognito window (or similar).
The xmb list and the ps store list are the same list afaik. So there's no need to check both.
Anyway, I'd suggest downloading the dlc bundle again.
The file should be called:
"network pass the last of us" or similar.
Make sure there's three map packs in the download list. Download those again as well just to be sure.
In which region/country are you located? Europe, the US, or? There could be regional differences to the bundle.
Btw, do you get any error message(s) when trying to access Factions?

Strange. Downloading "the last of us multiplayer (unlock)" should work.
What kind of error message are you getting (if any)? Tell me exactly what happens when you try to connect to Factions.
What about the map packs (I think there are three) ?
Try downloading other stuff from the "view downloads" or "download list" that seem to be mp related (other than weapons, skills, hats, etc.). There may be a "season pass" or similar there that may give you access to to Factions.
Though, try only to download stuff one dlc (or a handful at the most) on each try.
Also, since you seem to be new to factions on the ps3:
The PS3 freezes while trying to connect normally to Factions (the multiplayer)
This issue is caused by the Facebook support/feature being removed from the PS3 system.
This is a work-around for how to connect to Factions on the PS3 without hanging/freezing it:
1. Select Factions and then click the PS-button asap on your controller (i.e. switch/exit to xmb) while it's connecting.
2. Wait at least 10 seconds, and then press the PS-button again.
3. You should now be able to access the multiplayer.
Yes, a similar thing happened to me.
It seems the ps3 can't handle installing/activating all those dlcs at once.
Most of them I had to download again, manually, and individually, by going to "view downloads" in the ps store on the ps3. One can also go to "account management" > "transaction management" > "download list" in the xmb (after logging in to psn).
It's annoying, I know, but just locate the online/multiplayer pass in the list, download it again and it should now work. You'll probably also notice that a lot of the other dlcs are a still locked. So, to unlock those; rinse and repeat the same procedure again for each item you want unlocked. The only real important stuff are the weapons and skills, so focus on that when going through the list.
